Urinary Glucose Positivity in Cats: Does It Mean Diabetes?

Under normal circumstances, urine contains little to no glucose, making it undetectable in routine examinations. However, heightened excitement, excessive glucose or fructose ingestion, and consumption of carbohydrate-rich diets can elevate blood glucose levels beyond the renal threshold, resulting in glucose spillover into the urine.
Diabetes mellitus ranks as the second most common endocrine disorder in cats, with a higher prevalence among middle-aged to senior felines and a slight male predisposition. Although precise incidence rates remain unknown, the escalating numbers of overweight and obese cats contribute to a staggering rise in diabetic cases. Notably, cats weighing over 1.5 kilograms above their ideal body weight are deemed obese, indicating a significantly elevated risk of type 2 diabetes mellitus, particularly among domestic cats weighing 6 kilograms or more.
Diabetes mellitus, a pancreatic disorder, arises from the pancreas's inability to regulate blood glucose levels adequately. The pancreas comprises two distinct cell types with vastly different functions: one group produces enzymes essential for proper digestion, while the other, known as β-cells, secretes the hormone insulin.
Insulin binds to cell receptors, signaling them to absorb glucose appropriately, thereby providing crucial fuel to cells in fat, liver, and muscle tissues while lowering blood glucose levels. Simply put, diabetes mellitus stems from pancreatic dysfunction in glucose regulation.

The four primary symptoms of diabetes mellitus in cats include increased thirst, augmented urination, weight loss, and heightened appetite. Due to cats' inherent nature, these signs may go unnoticed, particularly during the early stages of the disease or when cats spend extended periods outdoors. Cats fed canned or semi-moist diets derive ample moisture from their food, making it harder to identify increased water intake. As the condition progresses, cats may exhibit decreased appetite, lethargy, weakness, unsteady gait, and, in advanced cases, severe symptoms such as thinning skin prone to injury, jaundice, coma, acute inflammation, and more.
Additional potential manifestations encompass abdominal enlargement (hepatomegaly), cataracts, and diabetic ketoacidosis.
To ascertain diabetes suspicion in cats:
Continuous elevation of glucose levels in both blood and urine needs to be demonstrated. Single blood glucose readings may not suffice for a definitive diagnosis in all cases due to stress-induced hyperglycemia.
The ultimate confirmation of feline diabetes may necessitate specialized testing, such as the serum fructosamine test, providing insights into the average blood glucose levels over the past 7-14 days. The primary goals of managing feline diabetes encompass: Restoring normal blood glucose concentrations.
Halting or controlling weight loss. Ceasing or minimizing signs of increased thirst and urination. Avoiding hypoglycemia induced by treatment. Treatment primarily revolves around a combination of insulin therapy and dietary modifications. Presently, injectable insulin stands as the mainstay of feline diabetes treatment. Unlike individuals with Type 2 diabetes mellitus, oral hypoglycemic agents like glipizide have not demonstrated sustained efficacy in cats. Various types of insulin preparations are available for feline diabetes treatment, such as lente insulin (Vetsulin), ProZinc, or glargine insulin. These insulin types differ in cost, duration of action, and concentration. It's crucial to note that each insulin type requires specific syringe sizes (U-100 or U-40). Insulin is administered subcutaneously approximately every 12 hours. While injections may appear daunting, most owners can easily learn to administer them at home, and cats often tolerate these injections well due to the tiny needle size. While the ideal injection interval should be 12 hours, adjustments within 1-2 hours are permissible if needed.
Low-carbohydrate diets have proven beneficial in improving blood glucose regulation in diabetic cats. Both dry and wet prescription foods are acceptable for this purpose (although wet food may yield slightly better results). Since clinically relevant postprandial hyperglycemia appears unlikely, it seems unnecessary to synchronize feeding times with insulin injections. In fact, to minimize potential diet-related blood glucose spikes, some clinical veterinarians prefer administering insulin while ensuring cats eat simultaneously, which may also prove more convenient for some owners. Low-carbohydrate diets are acceptable for cats prone to grazing throughout the day, especially if using long-acting insulin preparations. In cases where fasting is necessary for any reason (e.g., anesthesia procedures), administering 50% of the regular insulin dose and careful follow-up monitoring to ensure good glycemic control are typically advised.
Close monitoring by both owners and veterinarians is an integral component of feline diabetes management. Regular surveillance helps determine each cat's ideal insulin dosage and aids in avoiding complications such as hypoglycemia or uncontrolled diabetes and diabetic ketoacidosis. Regular assessments of weight, water intake, and appetite should be recorded to help gauge treatment efficacy and ensure therapeutic goals are met. |
